In general, the role of an elder care manager is to help our clients and families navigate the transitions of aging and ensure the proper guidance, resources, and support are in place. An effective eldercare manager acts like the coach or concierge to lead the client down the right path to aging well.

An ELITE Elder Care Manager is a nurse that conducts a detailed assessment that includes mental, physical, emotional, and psychological health. ELITE nurse provides medication management, physician referrals, manages and accompanies clients to physician appointments and communicates with the interdisciplinary team. In addition, ELITE elder care managers assess physical functionality, living conditions, socio-economic environment, social circles, family involvement, and any other key factors that may assist in determining the quality of life.
