Our Community Health Services support some of the most vulnerable in our community, including individuals experiencing homelessness, those with serious mental illness or substance use disorders, and others in need of extra care. We work closely with Medi-Cal programs to offer care coordination and housing support. With a team of 8 dedicated case managers, we help people find and keep stable housing, offering services like tenancy support, and day programs, and starting in 2024, help with rental deposits.
